Haven't done one, but I did see one eons ago that this post triggered a memory of. It had not only monthly pics (actually I think it was once a week) it had the child's favorite project or drawing they brought home from the week pictured as well. It was a lot like our 365 pages--it had a summary of what was learned that week, outings they may have had, and the photos varied from ones of just the child to some of the child in preschool or with a preschool friend. (they were all 4x6, and it was in a 4x6 two up photo album, two photos one of the child, one of the art, and two 4x6 journaling cards). One journal card was from the child's perspective, one from the parents

I've made books for each year after the 1st year that I do a two page spread per month. These layouts highlight mostly favorite photographs and photos that show important things of that month along with short journaling about what happened that month. I print these at home and put in an 8x8 album. I also have a few pocket pages in the back of each one that I stick important papers and that year's birthday cards. She will be 5 in two weeks and this has been a fairly easy way to keep up with it. My daughter loves to look at her books! I do tend to use lots of templates just to speed up the process. I think I will be printing these in a shutterfly book so I can have a copy too.
