Wow, I'm sure you're going to get a billion different responses here!
Since I'm a convert from paper scrapping, I kind of work from the bottom up.
I decide first which pictures I'm going to do.
Then I look around websites, in my favorites, and such, and find a page to scraplift. (I always use someone else's pages for inspiration. Sometimes I lift them, sometimes they're just a starting point.)
Then I choose my background and accent papers, lay my photos down, decide on frames/outlining/shadows.
Elements go on last, then journaling.
I'm anxious to see what everyone else does!
